John Sweetenham is Professor of Medicine, Cleveland Clinic Lerner College of Medicine of Case Western Reserve University. Michael Williams is Director of the Hematologic Malignancy Program, Hematology/Oncology Division and Cancer Center of the University of Virginia School of Medicine.
